INCOME AND EXPENSE DECLARATION
Important: You can complete the FLF Intake and Survey and bring with you or complete it when you check in at the front counter.
Complete the Income and Expense Declaration/FL-150 if you are asking for support. If asking for spousal support, also complete the Spousal Support Declaration Attachment/FL-157. Bring your last two months of pay stubs or if self-employed, your last Schedule C and a Profit and Loss Statement.
In addition to the forms, please bring 3 regular mailing envelopes, 2 manila envelopes (9x12) and 9 postage stamps for the court to return your copies.
